Candidacy
Most women that have had a tubal ligation are candidates for reversal, but there are a few important things we evaluate. Do you know if you are a candidate for this procedure?

Medical Review
We recommend that your operative note from your original surgery is reviewed before starting the scheduling process. Do you have an operative note?

Financial Readiness
Payment for surgery is due within 24 hours of scheduling your surgical date and a date must be chosen within 90 days of starting our scheduling process. Are you able to pay the full amount ($7,500+) within that time?
Yes, I am prepared to pay by wire, ACH, credit card or 3rd party financing (e.g., Care Credit) that I have already arranged
No, but I would like to lock-in my surgical quote by opening a prepayment account to save money towards a future scheduling date ($500 minimum contribution)
